Charter Oak Health Center

Charter Oak Health Center is committed to providing quality health care and continuity of care to all patients, regardless of their ability to pay. They offer a full range of medical, dental, and behavioral health services in the community where most of their patients live. In addition to primary care, they provide specialty services, dental care for adults and children, and outreach services to ensure their patients are compliant with their health plans. They also provide healthcare for the homeless at seven shelters and on a mobile medical van.
