Transitions LifeCare

Transitions LifeCare, formerly known as Hospice of Wake County, Inc., changed its name on April 30, 2014 to better reflect the full range of services it offers and its expanded service territory. The organization remains a local, non-profit providing physical, emotional, and spiritual care to individuals with advanced illnesses, their caregivers, and those who have lost a loved one. The rebranding does not change the organization's mission or commitment to serving patients regardless of their ability to pay. Additionally, Horizons Home Care, a division of Transitions LifeCare, offers a range of health care services to individuals needing medical assistance at home, including nursing care, therapy programs, and coordination of prescribed medications.
